To make an appointment, there are two steps:

In-person options:

2. Pre-pay with credit card via PayPal on this site.

Long distance options:

1. Pre-pay with a check before your scheduled appointment.
2. Money transfer, i.e., wire funds, Western Union.
3. Pre-pay with credit card via PayPal on this site.
